The Declarations

The following will now be required on all reports produced after 1st January 2018. An updated Diagnostic Assessment Report ProForma is downloadable from this SASC website.  This should be used for all reports produced for the Disabled Students' Allowance (DSA) or other application. 

  • is a qualified specialist teacher holding an approved qualification and a current SpLD Assessment Practising Certificate /is HCPC registered (delete one which doesn't apply). 
  • certifies that this assessment has been conducted and the report written in accordance with the SpLD Working Group 2005/DfES Guidelines for Assessment of SpLDs in Higher Education and subsequent updates
  • has personally (i) administered in a confidential face-to-face setting, (ii) scored and (iii) interpreted all the tests used in this assessment.
